New Patients
Please arrive 15 minutes early to complete a bit of additional paperwork.
After we’ve received your prescription, we’ll email/text you a link to the paperwork so you can complete electronically on on paper, if you prefer.
Additionally, please plan to bring a list of all medications you are currently taking as well as your insurance card.
Cancellations or Reschedules
If you need to cancel or reschedule your appointment, we ask that you give us at least 24 hours so that we can accommodate all our visiting patients.
